Resolution No. 399 series of 2024

A RESOLUTION RATIFYING THE MEMORANDUM OF AGREEMENT ENTERED INTO BY AND BETWEEN THE MUNICIPAL GOVERNMENT OF TAYTAY, REPRESENTED BY HON. ALLAN MARTINE S. DE LEON, MPA AND RENAL MED DIALYSIS CENTER, INC. REPRESENTED BY ITS PRESIDENT, MARIA MARU A. BANAAG

WHEREAS, the Municipality of Taytay, Province of Rizal ("LGU-Taytay") has entered into a Memorandum of Agreement ("MOA") with Renal Med Dialysis Center, Inc ("Service Provider") through Sangguniang Bayan Resolution No. 217, Series of 2023 entitled, "A resolution authorizing the Municipal Mayor Allan Martine S. De Leon, MPA, to negotiate and enter into a memorandum of agreement ("MOA") for the Municipal Government of Taytay, province of Rizal, with Renal Med Dialysis Center, Inc. represented by its President, Maria Maru a. Banaag;

WHEREAS, the Service Provider offers the Municipal Government to make their dialysis treatment services available to qualified Taytayeño beneficiaries;

WHEREAS, said MOA implies that the Municipal Government of Taytay agrees to shoulder the co-pay expenses incurred by the qualified beneficiaries, ensuring a NO CASH OUT POLICY for qualified Taytayeños;

WHEREAS, the other terms and conditions of the said MOA have been duly reviewed and considered by the representatives of both parties;

WHEREAS, the Sangguniang Bayan deems it proper to ratify the Memorandum of Agreement that was signed last 24 January 2024 by LGU-Taytay and the Service Provider through their respective representatives;

NOW THEREFORE, BE IT RESOLVED, as it is hereby resolved, by the 12th Sangguniang Bayan of Taytay, Rizal, in session duly assembled, hereby RATIFIES THE MEMORANDUM OF AGREEMENT ENTERED INTO BY AND BETWEEN THE MUNICIPAL GOVERNMENT OF TAYTAY, REPRESENTED BY HON. ALLAN MARTINE S. DE LEON, MPA AND RENAL MED DIALYSIS CENTER, INC. REPRESENTED BY ITS PRESIDENT, MARIA MARU A. BANAAG.

RESOLVED FURTHER, that copy of this Resolution be furnished to all concerned for their information and appropriate action.

ADOPTED, this 17th day of April 2024, 6:19 PM at the Sangguniang Bayan Session Hall, Municipality of Taytay, Province of Rizal.
