A RESOLUTION RATIFYING THE MEMORANDUM OF AGREEMENT ENTERED INTO BY THE MUNICIPALITY OF TAYTAY REPRESENTED BY THE MUNICIPAL MAYOR HON. ALLAN MARTINE S. DE LEON, MPA, WITH BONG SANTIAGO MEMORIAL CHAPEL FOR THE PROVISION OF CASKET IN THE FORM OF PHP 5,000 – BURIAL ASSISTANCE TO QUALIFIED INDIGENT FAMILY BENEFICIARIES OF THE DECEASED IN THE MUNICIPALITY OF TAYTAY, RIZAL
WHEREAS, the 12th Sangguniang Bayan had adopted Resolution No. 349 series of 2024 entitled “A Resolution Authorizing the Municipal Mayor, Allan Martine S. De Leon, MPA, to sign and enter into a Memorandum of Agreement with Bong Santiago Memorial Chapel for the Provision of Caskets in the form of Php 5,000 – Burial Assistance to qualified Indigent Family-Beneficiaries of the Deceased in the Municipality of Taytay, Province of Rizal”;
WHEREAS, there is currently a Memorandum of Agreement between the Local Government of Taytay duly represented by Hon. Mayor Allan Martine S. De Leon and the Bong Santiago Memorial Chapel herein represented by Mrs. Norwima Santiago for the provision of funeral and burial services;
WHEREAS, Bong Santiago Memorial Chapel will provide caskets and other burial services to qualified indigent family beneficiaries of the deceased within the Municipality of Taytay, Rizal;
WHEREAS, the ratification of this Memorandum of Agreement will pave way on the implementation of the burial and funeral assistance of our Municipal Government to those qualified indigent family beneficiaries of the deceased;
WHEREAS, the August Body find the duly signed Memorandum of Agreement (MOA) worthy of its cause that would be helpful to the bereaved indigent family of the deceased;
NOW THEREFORE, BE IT RESOLVED, as it is hereby resolved, by the 12th Sangguniang Bayan of Taytay, Rizal, in session duly assembled, hereby RATIFIES THE MEMORANDUM OF AGREEMENT ENTERED INTO BY THE MUNICIPALITY OF TAYTAY REPRESENTED BY THE MUNICIPAL MAYOR HON. ALLAN MARTINE S. DE LEON, MPA, WITH BONG SANTIAGO MEMORIAL CHAPEL FOR THE PROVISION OF CASKET IN THE FORM OF PHP 5,000 – BURIAL ASSISTANCE TO QUALIFIED INDIGENT FAMILY BENEFICIARIES OF THE DECEASED IN THE MUNICIPALITY OF TAYTAY, RIZAL.
RESOLVED FURTHER, that copies of this Resolution be furnished to Bong Santiago Memorial Chapel and the Office of the Local Chief Executive as both parties indicated in the duly signed Memorandum of Agreement and other concerned offices for their information and guidance.
ADOPTED, this 21st day of February 2024, 3:33 PM at the Sangguniang Bayan Session Hall, Municipality of Taytay, Province of Rizal.
